Dr. Raji Krishnan MBBS, DGO, AMEC

With over 40 years of experience in General Practice, Raji is a tireless advocate for her patients, equitable community health and care. Dr Raji has an extensive background and interest in:

Dr Sylvia Nicholls MBBS, FRACGP, BSc

Dr Sylvia Nicholls is a GP who practices patient-centred
care. She provides information to her patients about their health conditions.
Dr Sylvia is enthusiastic about life-long learning and enjoys having a yarn
with her patients. She sees patients from all ages and stages, with a
particular focus on improving chronic disease management and the health of
older people.

Dr Laura Lim MBBS, FRACGP, DRANZCOG

Dr Laura moved from Singapore to Australia for medical training and has since completed her fellowship in WA, which she now calls her second home. She enjoys all aspects of general practice, with a special interest in women’s health and paediatrics. Passionate about holistic and preventative care, she strives to empower her patients to take charge of their health and wellbeing.

Dr Vivienne Chukwuneke MBBS, FRACGP, MRCGP

Dr Vivienne Chukwuneke is a UK-trained General Practitioner who relocated to Australia in 2020 and now calls Perth home. She is dedicated to providing holistic, patient-centred care, with a strong focus on supporting the long-term health and wellbeing of her patients.

Dr. Vivienne holds Membership of the Royal College of General Practitioners (MRCGP) in the UK and is a Fellow of the Royal Australian College of General Practitioners (FRACGP). Her clinical interests include women’s and children’s health, men’s health, mental health, and chronic disease management.

Outside of medicine, Dr. Vivienne enjoys spending quality time with her family and is a proud soccer mum—often found on the sidelines enthusiastically supporting her son’s team on Sunday mornings.

Jennifer Hall MNNP, MMHN, BN

Jennifer is a qualified Nurse Practitioner specialising in mental health. Jennifer has extensive mental health experience in the community, tertiary hospitals and emergency department settings working at Armadale, Bentley and Royal Perth hospitals. Jen grew up locally and continues to be a local member of the community.

Our Nurse Practitioners work collaboratively with our GP's, they can diagnose, prescribe, order tests and complete referrals. Jen is available for appointments relating to your mental wellbeing

Sarah Paediatric Nurse Practitioner

Sarah is a paediatric specialised Nurse Practitioner with extensive experience in child health. 

Sarah qualified as a paediatric nurse in the UK before coming to Australian and training to be a Nurse Practitioner in QLD.  Sarah has experience working in paediatric hospitals in Australia and overseas. 

As a Nurse Practitioner Sarah can diagnose, refer, order tests and prescribe medications.

Our nurses have extensive experience in chronic disease management, medical procedures, vaccines, infusions and much more.

Kate Fulford. MPharm, BSc, GC-DIAB

Kate is our Associate Director overseeing the practice's strategic direction and implementing innovative models of care to improve patient outcomes. Kate is a qualified pharmacist with experience in General Practice, homeless health, Aboriginal Health and Aged Care. Kate also has a graduate certificate in diabetes management and a background in health innovation.

We have very experienced medical admin who support our entire team, they are here to help you navigate and coordinate your appointments. Our receptionists have a variety of experience some training themselves to be health professionals and other very experienced in managing health services. Our reception team is led by Paulina, who is a qualified EN and has experience in homeless health and Aboriginal Health.
